The Revance Aesthetics Patient "From Awards to Savings" Limited Time Offer (the "Offer"), owned and operated by Revance Therapeutics, Inc. ("Revance," "we," "us" or "our") is provided to you ("you," "your," or "user") under the following terms, including the amendments thereto, our Privacy Policy (available at https://www.revance.com/privacy-policy/ ), our Website Terms and Conditions (available at https://www.revance.com/terms-conditions/ ), and with any additional terms and conditions, rules, or polices that may be applicable to particular offerings made in connection with the Offer (collectively, the "Offer Conditions"). Your failure to follow the Offer Conditions may result in termination of your access to the Offer and all Rebates you have accrued in the Offer, without notice, in addition to Revance's other remedies. Your access to and use of the Offer is subject to the Offer Conditions and all applicable laws and regulations, including the laws and regulations governing copyrights and trademarks. The Offer Conditions shall supersede all previous Offer rules and/or terms and conditions. The following Offer Conditions may be modified by Revance from time to time, without advanced notice. Any such changes shall be reflected in an updated version of the Offer Conditions. BY ACCESSING THE OFFER, YOU ACCEPT, WITHOUT LIMITATION OR QUALIFICATION, THE OFFER CONDITIONS.

I. Description of the Offer

The Offer is a digital Offer where registered eligible users of the Offer may receive a rebate from use of DAXXIFY® and/or the Teoxane RHA® Collection aesthetics treatment ("offer") at participating healthcare provider offices. The Offer is not applicable for therapeutic uses of DAXXIFY®. You will scan a QR code in your healthcare provider’s office that will take you to our ‘From Awards to Savings’ landing page. You will be asked to complete our form and provide your personal information; enter in a unique code you receive from your healthcare provider, as well as proof of treatment for DAXXIFY® and/or the Teoxane RHA® Collection. The Offer may be changed from time to time at Revance's sole discretion without advance notice. Any change to the Offer or these Terms and Conditions will be posted on this site and by continuing to participate in the Offer, you are deemed to have agreed to the amended version of these Offer Conditions and are bound by any such revisions and should therefore periodically visit this website to review the current Offer Conditions to which you are bound.

II. Offer Eligibility

The Offer is open to legal residents of the United States who are 18 years of age or older at the time of treatment. The Offer is available for treatments between June 7, 2025 and October 31, 2025. Offer is available only at participating healthcare provider offices. You can earn a maximum of three consumer saving codes for a total amount of one hundred and fifty dollars ($150) in savings with this promotion. You should confirm office participation in advance of any treatment. Void where prohibited or restricted by law. All treatments with DAXXIFY® and Teoxane RHA® Collection must be purchased within the United States and from a participating provider's office.

III. No Medical Advice

WE CANNOT ANSWER UNSOLICITED E-MAILS REQUESTING PERSONAL MEDICAL ADVICE. IV. Reservation of Rights

The Offer and its Rebates are offered at Revance's discretion, and Revance has the right to modify or discontinue, temporarily or permanently, the Offer, including the Rebate offered, in whole or in part, for any reason, at our sole discretion. Revance may, among other options, withdraw, limit, modify, or cancel any Rebate; change the value of any Rebate; increase the requirements for any Rebate; or modify or condition the redemption of Rebate. You agree that Revance will not be liable to you or any third party for any modification or discontinuance of the Offer or any Rebate offered, in whole or in part.

V. Registration and Enrollment

To participate in the Offer, you first must have a treatment of DAXXIFY® and/or Teoxane RHA® Collection at one of our participating locations. At time of check out, your provider will share with you a QR code. You will need to scan that QR code with your personal mobile device, that will direct you to our ‘From Awards to Savings’ landing page. You will then be prompted to provide your personal information, enter in a unique and valid savings code (the code will be provided to you by the participating provider’s office) and then upload your receipt of the DAXXIFY® and/or Teoxane RHA® Collection treatment that includes the date of service and product administered.

Each Participant must provide a valid, unique, and non-used savings code at the time of registration. Duplicate or previously redeemed codes will not be accepted. The savings code must be used in accordance with the treatment guidelines and terms outlined in the Promotion.

Upon successful submission of all required documentation; including personal information, valid savings code, and qualifying treatment receipt; Participant information will be submitted to a third-party reimbursement vendor for reimbursement.

Please note: The distribution process by the reimbursement vendor may take between 5 to 15 business days from the date of submission.

No rebate will be issued until:

• The Participant’s information has been reviewed and verified as complete and accurate;
• The submitted treatment has been confirmed as qualifying under the Offer; and
• The submitted savings code is validated and approved for redemption.

The Rebate is available on most U.S. mobile carriers. Certain carriers, voice over IP (VoIP) and internet generated phone numbers are ineligible to participate in the Rebate. Revance reserves the right to stop service of the Offer on any carrier.

In consideration of your use of the Offer, you agree to provide true, accurate, current, and complete information about yourself as prompted by the online registration form and to maintain and promptly update your personal data to keep it true, accurate, current, and complete.

If you provide any information not true, inaccurate, not current, or incomplete; or if we have reasonable grounds to suspect that such information is not true, inaccurate, not current, or incomplete, we have the right to refuse any and all current or future use of the Offer (or any portion thereof) by you.

Once verification is complete, the rebate will be processed and issued via email, to the email address listed in the submission process, and to a digital gift card link. Rebates are subject to applicable terms and may be subject to limits per Participant and/or per treatment. Once you receive the digital gift card link, you have until December 31 of the following year; 12/31/26 to click “Activate”. Once you click “Activate” on the bank website, you’ll be prompted to enter your name and address to claim your Mastercard and your card number will be displayed along with the expiration date, which will be 6 months from the date of activation.

VI. Earning and Redeeming Offer Rebates

You will earn a $50 Rebate in the form of a digital gift card that can be used as a savings off your treatment with DAXXIFY® and/or Teoxane RHA® Collection when you receive treatment at an eligible healthcare professional's office which is enrolled in the Revance Offer. The Offer is only available through participating healthcare professionals' offices. Participating healthcare provider offices will let you know if they are offering the Rebate.

VII. Use of Your Information and Personal Information

The information that you provide in connection with the Offer will be used by Revance in connection with the administration of the Offer and to provide you with information about the Offer. Furthermore, Revance may use any information you provide for Revance's internal purposes and to send you information about Revance and its products and services. Revance will not share your information with any third party, except as necessary for the administration of the Offer or as required by law or legal process. If you do not wish Revance to send information to you about the Offer and Revance and its products and services, you should not participate in the Offer. By participating in the Offer, you agree that we may collect and use personal information about you that you provide to us in accordance with our Privacy Policy which can be found at: https://www.revance.com/privacy-policy/ . By using the Offer, you agree to be bound by the terms of our Privacy Statement.

VIII. Fraud

Fraud or abuse relating to the accrual or redemption of Offer Rebates may result in forfeiture. You are solely responsible for any fraudulent use of your Discounts. The Offer is provided to individual users who are natural persons for their own personal use. IT WILL BE CONSIDERED FRAUDULENT AND A VIOLATION OF THESE TERMS AND CONDITIONS FOR YOU TO SEEK COMPENSATION FOR REDEEMING BENEFITS THAT WERE NOT PROPERLY EARNED OR REDEEMED.

IX. Limitation of Liability; Release

Revance and its affiliates and their respective representatives, agents, directors, officers, shareholders, and employees ("Revance Entities") are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, inaccurate, incomplete, or unintelligible entries, submissions, or materials supplied in connection with Rebates; (ii) any injuries, losses, or damages of any kind arising in connection with or as a result of the Rebate, delivery or failure to timely deliver, possession, or use of the Rebate, or from participation in the Offer; or (iii) any printing or typographical errors in any materials associated with the Offer. Further, in no event shall the Revance Entities be liable for any damages of any kind or nature, including but not limited to, direct, indirect, incidental, consequential, exemplary, special (including loss or profit), punitive, or other damages arising from or in connection with the existence or use of the Offer or any such dispute, regardless of whether any of the Revance Entities has been advised as to the possibility of such damages.

X. Violation of the Offer Conditions

You understand and agree that in our sole discretion, and without prior notice, we may remove Rebates as a penalty or terminate your access to the Offer if we determine that you have violated our Offer Conditions.

XI. Proprietary Rights

You should assume that all contents of this site are copyrighted unless otherwise noted and may not be used except as provided herein and without our express written permission. Except as expressly provided herein, nothing contained herein shall be construed as conferring any license or right under any Revance copyright. All product names, logos, and service marks displayed on this site that are identified by® or ™ or appearing in type form different from that of the surrounding text (collectively, the "Trademarks") are registered or unregistered trademarks owned by or licensed to Revance or our affiliates, unless otherwise identified as being owned by another entity. Nothing contained herein shall be construed as conferring by implication, estoppel, or otherwise any license or right, either express or implied, under any patent or Trademark of Revance or any third party. No use of any Trademark may be made without our prior written authorization.

XII. Governing Law & Disputes

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of California without regard to choice of law principles. You agree that any dispute related to the Offer shall be submitted for resolution exclusively to the jurisdiction of the United States District Court of California. All applicable federal, state, and local laws and regulations apply. The invalidity or unenforceability of any provision of these Terms and Conditions shall not affect the validity or enforceability of any provision. In the event that any provision of these Terms and Conditions is found to be invalid or unenforceable, these Terms and Conditions shall be construed in accordance with their terms as if the invalid or unenforceable provision was not contained therein.

The Revance Aesthetics Patient Savings Limited Time Offer (the "Offer"), owned and operated by Revance Therapeutics, Inc. ("Revance," "we," "us" or "our") is made available to consumers interested in treatment with DAXXIFY® and RHA® Collection. As a healthcare professional ("HCP," "you," "your," or "user") you may participate in the Offer under the following terms and conditions, including the amendments thereto and with any additional terms and conditions, rules, or polices that may be applicable in connection with the Offer (collectively, the "Terms and Conditions"). Your failure to follow the Offer's rules, whether listed below or in supplemental notices posted at various points in the Offer, may result in termination of your access and ability to participate in the Offer, without notice, in addition to Revance's other remedies. Your participation in, access to, and use of the Offer is subject to the terms and conditions set forth herein and all applicable laws and regulations, including the laws and regulations governing copyrights and trademarks. The Terms and Conditions shall supersede all previous Offer rules and/or terms and conditions applicable to healthcare professionals. Any change to the Offer or these Terms and Conditions will be posted on this site and by continuing to participate in the Offer, you are deemed to have agreed to the amended version of these Offer Conditions and are bound by any such revisions and should therefore periodically visit this website to review the current Offer Conditions to which you are bound.

II. Offer Eligibility for HCPs

The Offer is open to appropriately licensed HCPs in the United States. To participate in the Offer, HCP must purchase at least 50 vials of DAXXIFY® and/or 35 boxes of Teoxane RHA® Collection in a single order at the ship-to location during the promotion period. Upon meeting the purchase requirement, you will receive either 30 or 60 unique rebate codes (dependent on if you purchase both DAXXIFY® and/or Teoxane RHA® Collection).

To qualify, the vials of DAXXIFY® and boxes of RHA® Collection must be purchased directly from Revance. Accounts cannot apply previous purchases made before the Promotional dates.

Accounts Eligibility and Order Aggregation Clause:

Participation in this Promotion by national account is strictly limited to those entities that have been pre-approved and designated by Revance prior to the commencement of the Promotion period. Eligibility was determined based on a combination of the following criteria: (1) the number of individual locations operating under the national account’s umbrella, and (2) the aggregate volume of orders placed by such locations with our brand of products, within the last six (6) months preceding the Promotion start date.

Qualified national accounts may aggregate orders from multiple participating locations under their network to meet the minimum purchase thresholds required by the Promotion. Only orders submitted by locations formally affiliated with the pre-approved national account, as documented in Revance records, shall be eligible for aggregation and inclusion toward promotional qualification.

Revance reserves the right to, in its sole discretion, to verify eligibility and to disqualify any entity or order that does not comply with these requirements.

III. The Offer is available for aesthetics treatments received by patients between July 7, 2025 and October 31, 2025. Void where prohibited or restricted by law. Reservation of Rights

IV. Earning and Redeeming Patient Offer Benefits

Patients will earn $50 Rebate(s) due to their treatment of DAXXIFY® and Teoxane RHA® Collection. The number of $50 Rebates provided each patient is at the sole discretion of the health care provider, although there is a maximum amount of three (3) rebates that is allowed per consumer, for a total of one hundred and fifty dollars ($150) in total savings available to each consumer. The HCP's office must have made a qualifying purchase order to qualify – purchase a minimum of 50 vials of DAXXIFY® and/or 35 boxes of Teoxane RHA® Collection on a single order at the ship-to location between July 7, 2025 through the promotion availability. After making qualified purchases of DAXXIFY® and/or Teoxane RHA® Collection as described above. After treating patients with DAXXIFY® and/or Teoxane RHA® Collection, during the checkout process, you will need to share the QR code provided to you from your Revance Professional Aesthetic Consultant or available on Allego and Marketing Hub. You or your staff should then instruct the patient to scan the QR code with their personal mobile device, that will direct them to our ‘From Awards to Savings’ landing page. The patient will then be prompted to provide their personal information, enter in a unique and valid rebate code (you or your staff will need to provide the patient) and then lastly the patient needs to upload receipt of their DAXXIFY® and/or Teoxane RHA® Collection treatment that includes the date of service and product administered.

Revance will reimburse the patient $ 50 for each rebate code and receipt of treatment submitted. Reimbursement of rebates to patients will be digitally emailed to them within 5 to 15 business days. Practices can redeem codes with eligible patients through October 31, 2025. After October 31, 2025, unused codes will be invalid.

V. Use of Your Information and Personal Information

The information that you provide in connection with the Offer will be used by Revance in connection with the administration of the Offer and to provide you with information about the Offer. Furthermore, Revance may use any information you provide for Revance's internal purposes and to send you information about Revance and its products and services. Revance will not share your information with any third party, except as necessary for the administration of the Offer or as required by law or legal process. If you do not wish Revance to send information to you about the Offer and Revance and its products and services, you should not participate in the Offer. XI. Product Insurance Exclusions

This Patient Rebate Offer applies only to private patients and may NOT be combined with any other promotional offers, rebates, discounts, or benefit programs. This Offer is also NOT to be used if the patient's treatment is paid for, or reimbursed by, Medicare, Medicaid, VA, DoD, TRICARE, commercial insurance, employer insurance plans, or any comparable federal or state programs. Patients who are being treated with product that is covered by any insurance for any treatment of the same or similar condition are not eligible for this discount.

XII. HCP Compliance

As an HCP, you agree that your participation in this program is consistent with your professional obligations, applicable codes of professional conduct, and all applicable federal, state, and local laws, rules, and regulations, including without limitation all healthcare related laws and regulations. This Offer is not intended to be an inducement to prescribe, purchase, or recommend any particular treatment or medical product.

XIII. Limitation of Liability; Release

Revance and its affiliates and their respective representatives, agents, directors, officers, shareholders, and employees ("Revance Entities") are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, inaccurate, incomplete, or unintelligible entries, submissions, or materials supplied in connection with benefits; (ii) any injuries, losses, or damages of any kind arising in connection with or as a result of the benefit, delivery or failure to timely deliver, possession, or use of the benefit, or from participation in the Offer; or (iii) any printing or typographical errors in any materials associated with the Offer. Further, in no event shall the Revance Entities be liable for any damages of any kind or nature, including but not limited to, direct, indirect, incidental, consequential, exemplary, special (including loss or profit), punitive, or other damages arising from or in connection with the existence or use of the Offer or any such dispute, regardless of whether any of the Revance Entities has been advised as to the possibility of such damages.

XIV. Modification/Termination

Revance reserves the right to modify, suspend, cancel, or terminate this Offer at any time, for any reason, without prior notice. Revance is not responsible for any costs, expenses, or other liability that may be incurred as a result of participation in this Offer.
