Revance Aesthetics Patient “Beauty of Savings” Limited Time Offer

The Revance Aesthetics Patient “Beauty of Savings” Limited Time Offer (the “Offer”), owned and operated by Revance Therapeutics, Inc. (“Revance,” “we,” “us” or “our”) is provided to you (“you,” “your,” or “user”) under the following terms, including the amendments thereto, our Privacy Policy (available at https://www.revance.com/privacy-policy/), our Website Terms and Conditions (available at https://www.revance.com/terms-conditions/), and with any additional terms and conditions, rules, or polices that may be applicable to particular offerings made in connection with the Offer (collectively, the “Offer Conditions”). Your failure to follow the Offer Conditions may result in termination of your access to the Offer and all Discounts you have accrued in the Offer, without notice, in addition to Revance’s other remedies. Your access to and use of the Offer are subject to the Offer Conditions and all applicable laws and regulations, including the laws and regulations governing copyrights and trademarks. The Offer Conditions shall supersede all previous Offer rules and/or terms and conditions. The following Offer Conditions may be modified by Revance from time to time, without advanced notice. Any such changes shall be reflected in an updated version of the Offer Conditions. BY ACCESSING THE OFFER, YOU ACCEPT, WITHOUT LIMITATION OR QUALIFICATION, THE OFFER CONDITIONS.
I. Description of the Offer
The Offer is a text, mobile device-based Offer where registered eligible users of the Offer may receive up to $115.00 discount off DAXXIFY® and RHA® Collection aesthetics treatment (“Discount”) at participating healthcare provider offices. The Offer is not applicable for therapeutic uses of DAXXIFY®. You will receive a code from your healthcare provider which you will send by text to receive the Discount. The healthcare provider’s office will apply the Discount to reduce the total cost of your DAXXIFY® and RHA® Collection treatment up to $115.00 (single brand treatment discounts: DAXXIFY ® $40.00 and RHA® Collection $75.00). The Offer has no independent cash or monetary value and is not redeemable for cash, transferable or assignable for any reason, and cannot be sold, traded, bartered, auctioned through an online auction site or otherwise; any such Discount may be confiscated by Revance and/or cancelled. The Offer may be changed from time to time at Revance’s sole discretion without advance notice. Any change to the Offer or these Terms and Conditions will be posted on this site and by continuing to participate in the Offer, you are deemed to have agreed to the amended version of these Offer Conditions and are bound by any such revisions and should therefore periodically visit this website to review the current Offer Conditions to which you are bound.
II. Offer Eligibility
The Offer is open to legal residents of the United States who are 18 years of age or older at the time of treatment. The Offer is available for treatments between June 2, 2024 and September 30, 2024. Offer is available only at participating healthcare provider offices. You should confirm office participation in advance of any treatment. Void where prohibited or restricted by law. Only one Discount is allowed per person during the promotion period. All treatments with DAXXIFY® and RHA® Collection must be purchased within the United States and from a participating provider’s office.
III. No Medical Advice
THE INFORMATION CONTAINED WITHIN THIS OFFER, ON THIS SITE, OR IN ANY EMAIL OR TEXT MESSAGE OR OTHER CORRESPONDENCE SENT TO YOU BY REVANCE IS NOT MEDICAL ADVICE AND IT IS NOT INTENDED TO BE MEDICAL ADVICE. WE CANNOT ANSWER UNSOLICITED E-MAILS REQUESTING PERSONAL MEDICAL ADVICE. IV. Reservation of Rights
The Offer and its Discounts are offered at Revance’s discretion, and Revance has the right to modify or discontinue, temporarily or permanently, the Offer, including the Discounts offered, in whole or in part, for any reason, at our sole discretion. Revance may, among other options, withdraw, limit, modify, or cancel any Discount; change the value of any Discounts; increase the requirements for any Discount; or modify or condition the redemption of Discounts. You agree that Revance will not be liable to you or any third party for any modification or discontinuance of the Offer or any Discount offered, in whole or in part.
V. Registration and Enrollment
In order to participate in the Offer, you first must register and enroll by following the text prompts. The participating provider will provide you with the keyword SAVINGS to text to the dedicated short code 78457. By texting SAVINGS to 78457, you initiate the Offer registration process and receive a series of about 5-10 text messages that provide instructions or request for information such as your name, email address and treatment received. You will receive texts that provide you with links to the Offer Terms & Conditions, links to DAXXIFY® Prescribing Information; and you will be prompted to text a unique code for your Offer and will receive a text that either confirms your code and Offer, reports an error or that the code is invalid and requests another code. The code will be provided to you by the participating provider’s office.
The Offer is available on most U.S. mobile carriers. Certain carriers, voice over IP (VoIP) and internet generated phone numbers are ineligible to participate in the Offer. Revance reserves the right to stop service of the Offer on any carrier.
In consideration of your use of the Offer, you agree to provide true, accurate, current, and complete information about yourself as prompted by the online registration form and to maintain and promptly update your personal data to keep it true, accurate, current, and complete.
If you provide any information not true, inaccurate, not current, or incomplete; or if we have reasonable grounds to suspect that such information is not true, inaccurate, not current, or incomplete, we have the right to refuse any and all current or future use of the Offer (or any portion thereof) by you. Only one Offer allowed per patient during the promotion period.
You can cancel the SMS service at any time. Just text “STOP” to the short code 78457. After you send the SMS message “STOP” to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carriers are not liable for delayed or undelivered messages.
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive about 5-10 text messages. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
VI. Earning and Redeeming Offer Discounts
You will earn a Discount in the amount set by Revance off of your treatment with DAXXIFY® and RHA® Collection when you receive treatment at an eligible healthcare professional’s office which is enrolled in the Revance Offer. The Offer is only available through participating healthcare professionals’ offices. Participating healthcare provider offices will let you know if they are offering the Discount.
VII. Use of Your Information and Personal Information
The information that you provide in connection with the Offer will be used by Revance in connection with the administration of the Offer and to provide you information about the Offer. Furthermore, Revance may use any information you provide for Revance’s internal purposes and to send you information about Revance and its products and services. Revance will not share your information with any third party, except as necessary for the administration of the Offer or as required by law or legal process. If you do not wish Revance to send information to you about the Offer and Revance and its products and services, you should not participate in the Offer. By participating in the Offer, you agree that we may collect and use personal information about you that you provide to us in accordance with our Privacy Policy which can be found at: https://www.revance.com/privacy-policy/. By using the Offer, you agree to be bound by the terms of our Privacy Statement.
VIII. Fraud
Fraud or abuse relating to the accrual or redemption of Offer Discounts may result in forfeiture. You are solely responsible for any fraudulent use of your Discounts. The Offer is provided to individual users who are natural persons for their own personal use. IT WILL BE CONSIDERED FRAUDULENT AND A VIOLATION OF THESE TERMS AND CONDITIONS FOR YOU TO SEEK COMPENSATION FOR REDEEMING BENEFITS THAT WERE NOT PROPERLY EARNED OR REDEEMED.
IX. Limitation of Liability; Release
Revance and its affiliates and their respective representatives, agents, directors, officers, shareholders, and employees (“Revance Entities”) are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, inaccurate, incomplete, or unintelligible entries, submissions, or materials supplied in connection with Discounts; (ii) any injuries, losses, or damages of any kind arising in connection with or as a result of the Discount, delivery or failure to timely deliver, possession, or use of the Discount, or from participation in the Offer; or (iii) any printing or typographical errors in any materials associated with the Offer. Further, in no event shall the Revance Entities be liable for any damages of any kind or nature, including but not limited to, direct, indirect, incidental, consequential, exemplary, special (including loss or profit), punitive, or other damages arising from or in connection with the existence or use of the Offer or any such dispute, regardless of whether any of the Revance Entities has been advised as to the possibility of such damages.
X. Violation of the Offer Conditions
You understand and agree that in our sole discretion, and without prior notice, we may remove Discounts as a penalty or terminate your access to the Offer if we determine that you have violated our Offer Conditions.
XI. Proprietary Rights
You should assume that all contents of this site are copyrighted unless otherwise noted and may not be used except as provided herein and without our express written permission. Except as expressly provided herein, nothing contained herein shall be construed as conferring any license or right under any Revance copyright. All product names, logos, and service marks displayed on this site that are identified by ® or ™ or appearing in type form different from that of the surrounding text (collectively, the “Trademarks”) are registered or unregistered trademarks owned by or licensed to Revance or our affiliates, unless otherwise identified as being owned by another entity. XII. Governing Law & Disputes
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of California without regard to choice of law principles. You agree that any dispute related to the Offer shall be submitted for resolution exclusively to the jurisdiction of the United States District Court of California. All applicable federal, state, and local laws and regulations apply. The invalidity or unenforceability of any provisions of these Terms and Conditions shall not affect the validity or enforceability of any provision. In the event that any provision of these Terms and Conditions is found to be invalid or unenforceable, these Terms and Conditions shall be construed in accordance with their terms as if the invalid or unenforceable provision was not contained therein.

The Revance Aesthetics Patient Reward Limited Time Offer (the “Offer”), owned and operated by Revance Therapeutics, Inc. (“Revance,” “we,” “us” or “our”) is made available to consumers interested in treatment with DAXXIFY® and RHA® Collection. As a healthcare professional (“HCP,” “you,” “your,” or “user”) you may participate in the Offer under the following terms and conditions, including the amendments thereto and with any additional terms and conditions, rules, or polices that may be applicable in connection with the Offer (collectively, the “Terms and Conditions”). Your failure to follow the Offer’s rules, whether listed below or in supplemental notices posted at various points in the Offer, may result in termination of your access and ability to participate in the Offer, without notice, in addition to Revance’s other remedies. Your participation in, access to, and use of the Offer are subject to the terms and conditions set forth herein and all applicable laws and regulations, including the laws and regulations governing copyrights and trademarks. II. Offer Eligibility for HCPs
The Offer is open to appropriately licensed HCPs in the United States. To participate in the Offer, HCP must purchase at least 35 vials of DAXXIFY® and 35 boxes of RHA® Collection in a single order at the ship-to location during the promotion period. Upon meeting the purchase requirement, you will receive at minimum fifty-five $115.00 patient Discount for your patients, no charge product and access to educational opportunities. See purchasing levels below:
In order to qualify, the vials of DAXXIFY® and boxes of RHA® Collection must be purchased directly from Revance. Accounts cannot apply previous purchases made before the Promotional dates. Practices cannot combine smaller orders to qualify. Practices with multiple locations cannot combine smaller orders made at different locations to qualify. Practice must purchase at minimum 35 vials of DAXXIFY and 35 boxes of RHA® Collection at the ship-to location on a single order (maximum of 3 qualifying purchases during buy-in period). Only one Discounts is available per patient during the promotion term.

Patients will earn a Discount up to the amount of $115.00 off their treatment of DAXXIFY® and RHA® Collection (DAXXIFY ® $40.00 and RHA® Collection $75.00). The HCP’s office must have made a qualifying purchase order to qualify – purchase a minimum of 35 vials of DAXXIFY and 35 boxes of RHA® Collection on a single order at the ship-to location between June 1, 2024 through June 27, 2024 (see chart above for all buy-in levels) After making qualified purchases of DAXXIFY® and RHA® Collection as described above, you will receive patient Discount codes to provide patients which the patient will text to Revance. Once Revance confirms the code, the patient will be entitled to a $115.00 discount off the patient’s treatment of DAXXIFY® and RHA® Collection (DAXXIFY ® $40.00 and RHA® Collection $75.00).
Revance will reimburse the practice for the redemption codes with no charge product shipped to the account. The reimbursement product includes a minimum of 5 boxes of RHA® Collection and 8 vials of DAXXIFY®. See chart above that includes the amount of reimbursement product at each buy-in level. Reimbursement product will be shipped out 2 business days after qualifying promotion purchase. Practices can redeem codes with eligible patients through September 30, 2024. After September 30, 2024, unused codes will be invalid.
V. Use of Your Information and Personal Information
The information that you provide in connection with the Offer will be used by Revance in connection with the administration of the Offer and to provide you with information about the Offer. Furthermore, Revance may use any information you provide for Revance’s internal purposes and to send you information about Revance and its products and services. Revance will not share your information with any third party, except as necessary for the administration of the Offer or as required by law or legal process. If you do not wish Revance to send information to you about the Offer and Revance and its products and services, you should not participate in the Offer. By participating in the Offer, you agree that we may collect and use personal information about you that you provide to us in accordance with our Privacy Policy which can be found at: https://www.revance.com/privacy-policy/. By using the Offer, you agree to be bound by the terms of our Privacy Statement.
VI. Fraud
